Where you can display booking forms?
You can show booking widgets on:
Product pages
App scheduling page (Custom scheduling pages you create with Slot Appointment app)
Cart page
Other pages (Pages which you create on Online store → Pages in your Shopify admin)
How you can display booking widgets
And, there are 2 displaying options:
Booking widget as a button, which opens a scheduling form in pop-up.
Embedding scheduling form directly on pages (product pages, app scheduling page, cart page, other pages)

Setting display options & pages
You can set different display options on different pages.
To set display options for different pages, on your Shopify admin:
Navigate to Widget customization → Widget options
Here, you can select how you want to display the booking forms from the dropdown menu:
Notes:
Widgets get displayed, when corresponding pages are configured at scheduling pages.
If the widget is displayed on selected product pages, they should be added and switched on at scheduling pages.
If the widget is displayed on all product pages, you may need to edit certain product pages and assign them appointment types. If no appointment type is selected for a product, all active appointment types will be used.
